Third Party Authorization Form
If you would like to authorize a third party to be able to discuss your file with Brock & Scott please complete this form and return to our office. Please allow 48–72 hours for review. All information requested on the form must be filled in.

Electronically Signed Third-Party Authorization Forms
If you submit a third-party authorization form that was signed electronically using a third-party e-signature service (such as DocuSign, Adobe Sign, or a similar platform), you must also provide the e-signature service's Certificate of Completion (or equivalent audit report).
The Certificate of Completion must include, at a minimum:
- Signer's email address.
- Dates and times of delivery, viewing, and signing.
- IP address used by the signer.
- Authentication method used.
- Complete event history.
- Evidence that electronic signature (ESIGN) consent was obtained.
- Tamper-seal confirmation showing the document has not been altered after signing.
